About the Role
We are seeking a Nurse Practitioner to provide on-site rounding services in nursing homes/skilled nursing facilities, with a primary focus on treating
a combination of acute and chronic pain syndromes.
This role is ideal for NPs who enjoy independent practice, value consistent schedules, and are comfortable managing complex, medically fragile populations.
What You’ll Do
- Conduct regular on-site rounds in assigned facilities
- Evaluate and manage acute and
chronic pain conditions - Prescribe and manage medications, including controlled substances
- Coordinate care with facility staff (PT’s, RNs, DONs, administrators)
- Complete timely EMR documentation
Patient Population
- Acute rehabilitation and long-term care residents
- Geriatric patients with multiple comorbidities
- Chronic pain, mobility-related pain, and dementia-associated pain syndromes
Schedule & Work Environment
- Primarily weekday daytime hours
- No hospital shifts
- Travel between facilities is required in some cases
- Predictable patient census and rounding schedules
What We’re Looking For
- Active NP license in the practicing state
- DEA license (or ability to obtain)
- Experience or strong interest in:
- Geriatrics
- Long-term care
- Pain management
- Comfortable working independently and traveling locally
