Job Title: Senior Programs Care Coordinator

Reports to: Senior Programs Director

Employee Status: Full-Time, 40 hours/week, non-exempt

Rate: $24.00/hr

Work Setting: Work is performed from office in Morris, IL, with visits to client homes.

Potential hours of work may include occasional weekend hours.

SUMMARY:

The Grundy County Health Department works to preserve, protect and promote the health and well-being of Grundy County. Services are offered in response to community need and in accordance with oversight agencies to clients of the lifespan – infant to older adults.

The ideal candidate will compassionately serve the residents of Grundy County, working as a team player and cooperating with colleagues to best serve the residents in a fast-paced and ever-changing environment. Under the supervision of the Director of Senior Programs, the candidate will possess a positive attitude and be capable of critical thinking, sound judgement and working independently as needed.

The Grundy County Health Department is seeking an experienced coordinator who is passionate about assisting senior residents of Grundy. Ideal candidate will enthusiastically drive efforts to identify and inform persons 60 in Grundy County of available services and programs for which they may be eligible. This includes but is not limited to conducting daily home visits, assessing eligibility of Community Care Program, establishing a plan of care with client/family, performing Choices for Care Screenings, and completing required documentation as mandated by IDoA, AgeGuide, other funders, and the County Health Department.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

Include the following. Other duties may be assigned.

  • Conducts home visits with client(s) and family to assess for eligibility of Community Care Program (CCP), Federal and State Benefit Programs.
  • Provide weekend coverage of services, in accordance with IDoA requirements
  • Establish a plan of care in conjunction with client/family as needed
  • Assist client in filling out applications
  • Knowledgeable about resources and makes appropriate referrals to other agencies
  • Performs Choices for Care Screenings as needed.
  • Maintains regular contact with clients as mandated by IDoA, NEIL, and AoA.
  • Completes required documentation as mandated by IDoA, NEIL, AoA, GCHD, or other oversight agencies
  • Attend training as it is made available by IDoA, NEIl, AoA, and the GCHD
  • Attend supervisory, staff development, in-service and other meetings as required.
  • Projects good image, maintains credibility and integrity of agency.
  • Discuss client problems, such as depression, family crisis, health and financial matters to supervisor for short term support.
  • Take referrals and completes the intake form for all program referrals: HDM, CCP, Choices for Care, Case Management, 60 counseling, and all other programs and services related to Seniors.
  • Assists supervisor in organizing special events such as health fairs, outreach events, Older Americans Month activities, etc.
  • Must be able to lift up to 20lbs
  • Obtain Care Coordinator certification and SHIP certification for NPR entry
  • Abides by policies of the Board of Health and the Grundy County Personnel Code.
  • Performs other duties as required or assigned.

EMERGENCY RESPONSE

In times of public health emergency, you may be asked to report for specialized assigned duties inside or outside of Grundy County. On occasion must be willing to work an extended and flexible schedule and may be required to be available by phone and /or pager. May be asked to report to work at any time in response to an emergency. Perform emergency response roles, as determined by the situation.

QUALIFICATIONS

EDUCATION and/or EXPERIENCE

  • Prefer a RN, or a BSN or a BA/BS degree in social science, social work or related field. OR
  • An LPN with 2 years of program experience which is defined as assessment, provision, and/or authorization of formal services for the elderly. OR
  • Will consider one year of program experience, which is defined as assessment, provision, and/or authorization of formal services for the elderly; may replace on year of college education up to and including four years of experience replacing a baccalaureate degree.

LANGUAGE SKILLS

Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organizations. Bilingual Spanish speaking preferred.

MATHEMATICAL SKILLS

Ability to apply concepts of basic math.

REASONING ABILITY

Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions in written, oral, diagram, or schedule form.

CERTIFICATES, LICENSES, REGISTRATIONS

  • Required to complete and maintain certification as a CCP Case Manager as required by IDOA.
  • A valid driver’s license and proof of insurance are job requirements.
  • Required to complete and maintain SHIP certification for NPR entry.

PHYSICAL DEMANDS

While performing the duties of this job, the employee is occasionally required to stand; walk; use hands to finger, handle or feel; reach with hands and arms; climb or balance; stoop, kneel, crouch or crawl; talk or hear; and taste or smell as well as lift up to 25 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.
