Job Purpose
The Qualified MentalHealth Specialist (QMHS), also known as a Case Manager, is responsible for providing counseling, mentoring, and coaching to clients. The QMHS will conduct multiple weekly meetings with clients, focusing on their needs and goals, and will maintain accurate, detailed, and informative progress notes to document the clients’ progress.
Key Responsibilities
- Client Care:
- Meet with each client 3-5 times a week to assess their needs and progress.
- Develop and implement personalized plans to meet client goals.
- Provide counseling, mentoring, and coaching to clients to support their mental health and well-being.
- Documentation:
- Maintain accurate and up-to-date progress notes for each client.
- Ensure compliance with all documentation and reporting requirements.
- Collaboration:
- Attend and contribute to staff and case manager meetings to coordinate care and ensure alignment with organizational goals.
- Transport clients as needed to ensure access to services.
- Foster positive and professional relationships with coworkers, clients, and other stakeholders.
- Additional Duties:
- Perform other duties as required to support case management operations and organizational objectives.

Qualifications and Experience
- Bachelor’s degree in Social Work, Psychology, or a related field.
- Minimum of 2 years of experience in case management or a related field.
- Strong organizational and communication skills.
- Proficiency in case management software and compliance standards.

Role in Organizational Structure
- Reports directly to the Regional Director.
- Collaborates closely with other Case Managers (QMHS) and the Program Manager.
- Acts as a key link between clients and the broader organizational structure.
