Role Description
This is a full-time, on-site Assessment Nurse role based in La Mesa, CA. The Assessment Nurse will be responsible for conducting comprehensive health assessments, creating care plans tailored to individual needs, and updating these plans as needed. Daily responsibilities include monitoring residents' health and well-being, collaborating with healthcare providers and team members, providing education to residents and families, and maintaining accurate documentation in compliance with regulatory standards. Additionally, the Assessment Nurse will play a vital role in ensuring high-quality care for residents with memory care needs.
Qualifications
- A current and valid Registered Nurse (RN) or Licensed Vocational Nurse (LVN) license in California
- Experience in health assessment, care planning, and documentation
- Excellent nursing, monitoring, and clinical evaluation skills
- Ability to collaborate effectively with healthcare professionals, caregivers, and families
- Strong verbal and written communication skills, as well as a compassionate and patient-centered approach
- Familiarity with memory care or dementia care is highly beneficial
- Proficiency with healthcare-related software and electronic health records (EHR) is a plus
- Flexibility and adaptability to manage changing priorities in a fast-paced environment
