Job Overview
We are seeking a compassionate Family Nurse Practitioner to join our healthcare team. The ideal candidate will have experience in primary care, acute and chronic disease management, and a broad range of specialties, including geriatrics, pediatrics, behavioral health, and emergency medicine. This role involves providing comprehensive patient assessments, diagnosing illnesses, developing treatment plans, and coordinating care across various settings such as outpatient clinics, hospitals, nursing homes, and telehealth platforms. The Family Nurse Practitioner will utilize clinical skills and medical knowledge to deliver high-quality patient-centered care while adhering to HIPAA regulations and documentation standards. Experience with EMR/EHR systems like Epic, eClinicalWorks, and Athenahealth is essential for efficient patient management.
Responsibilities
- Conduct thorough physical examinations and patient assessments across diverse populations, including pediatrics, geriatrics, neonatal, and adult patients.
- Diagnose acute and chronic conditions utilizing diagnostic evaluation tools such as laboratory testing, medical imaging, and specimen processing.
- Develop and implement individualized treatment plans incorporating medication administration (including injections), IV infusion, catheterization, tube feeding, dermal fillers, botulinum toxin treatments, esthetic laser treatments, and other procedures within the scope of practice.
- Provide health coaching on nutrition, disease prevention (including family planning), discharge planning, utilization review & management.
- Document all patient interactions accurately using EMR/EHR systems like Epic or eClinicalWorks while maintaining compliance with HIPAA standards.
- Collaborate with multidisciplinary teams including specialists in cardiology (cardiac catheterization), ophthalmology (ophthalmology procedures), occupational health services, behavioral health providers, and more.
- Participate in telehealth consultations utilizing telehealth experience to extend care access remotely.
- Ensure infection control protocols are followed rigorously to prevent cross-contamination in all clinical settings.
Skills
- Proven expertise with EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks for efficient documentation.
- Strong clinical skills in medical-surgical nursing practices, including vital signs assessment, physical examinations, and medical terminology proficiency.
- Experience administering injections and performing IV insertions with aseptic technique.
- Ability to perform diagnostic evaluations using laboratory tests or medical imaging; familiarity with ICD-9/ICD-10 coding for accurate documentation.
- Skilled in managing complex cases involving acute pain management (including opioid stewardship), pain management techniques like nerve blocks or dermal fillers; familiarity with CPT coding for billing purposes.
- Excellent communication skills for patient education on health promotion strategies; experience working with diverse populations.
