POSITION SUMMARY:
The Community Health Coordinator plays a vital role in improving the health and well-being of our community by leading chronic disease prevention initiatives at the Greenwich Y and building strong partnerships to expand our outreach. Reporting to the Sr. Director of Member Experience, this position supports program instructors and engages current and prospective participants to ensure high-quality, inclusive programming. Key responsibilities include developing and maintaining collaborative relationships with local health organizations, driving referrals to the Y, and contributing to member satisfaction and retention through exceptional service. Sr. Director of Member Experience
PAY RANGE:
$30.00/hour
ESSENTIAL FUNCTIONS:
- Under the direction of the Sr. Director of Member Experience, the Community Health Coordinator is responsible for providing support and guidance to program coaches, participants and potential participants
- Applies a results-driven approach to growing our partnerships with area health providers and growing our chronic disease programs. Regularly tracking progress against established KPIs
- The Community Health Coordinator will also support relationships with key partners, participating in community events and driving referrals to the Greenwich YMCA
- Models inclusive behaviors and embraces all dimensions of diversity
- Supports the implementation of our chronic disease programs (ie., Diabetes Prevention Programs (DPP) and Livestrong), including participant recruitment, referrals, community outreach and data reporting
- Assist in fostering partnerships with key collaboration organizations and subject matter experts to expand the Y’s efforts to improve outcomes for underserved populations
- Establish and nurture relationships with community members, healthcare providers, and local community-based organizations to build a robust referral network
- Guide prospective chronic disease participants through enrollment processes, paperwork, and lead departmental event participation
- Deliver outstanding customer service while engaging with participants, YMCA staff, members, and diverse community stakeholders
- Develop strong, authentic relationships with community partners and participants by actively listening, showing empathy, and responding to their needs
- Complete training to become a coach for the National Diabetes Prevention Program and other chronic disease programs/initiatives
- Assists in communicating the objectives/services of the Chronic Disease Department and its individual initiatives to the public, community agencies, and professional groups
- Perform other duties as assigned in support of program and organizational goals
- Maintains confidentiality of information
- This role is a combination of in-person and home/office-based work

QUALIFICATIONS:
- Knowledge of chronic disease and evidence based programs
- Familiarity with local private and non-profit healthcare providers
- Exceptional communication and relationship-building skills to connect with diverse populations
- Self-starter, critical thinker, and owner of responsibilities
- Strong public relations skills with the ability to represent the organization positively
- Demonstrated organizational, planning, and problem-solving abilities
- Ability to exercise discretion and independent judgment
- CPR and First Aid certification
- Grant experience a plus
- Bilingual English and Spanish not required but a plus
