Position Summary

The Workforce Case Manager serves as a trusted guide for members of the Lifelong Skills and Opportunity Center, with a special focus on individuals aged 60 , those navigating behavioral health challenges, and neurodivergent populations. This role is dedicated to helping members identify barriers, access essential resources, and achieve personal and career goals. The Case Manager provides tailored support, connects members to workforce and community opportunities, and ensures they have access to holistic services that promote stability, independence, and long-term success.

Responsibilities

Member Support & Case Management

• Conduct screenings and assessments to determine members’ needs and develop individualized service plans.
• Provide/connect direct assistance with accessing community resources, including housing, food assistance, healthcare, tax support, and other benefits.
• Offer ongoing guidance, encouragement, and accountability to help members reach their employment and personal goals.

Administrative & Recordkeeping

• Maintain accurate case notes, files, and service records in compliance with LSOC standards.
• Track member progress and provide regular updates to leadership.
• Support outreach, recruitment, and program development efforts to strengthen LSOC’s presence and impact.

Collaboration & Partnerships

• Serve as a connector between LSOC, the Center for Healing, and the Center for Wellness, ensuring members can access wraparound services.
• Stay current on New York State programs, benefits, and workforce opportunities to keep resources relevant and up to date.
• Participate in team meetings and contribute to strategies that improve service delivery and outcomes.

Qualifications

• Bachelor’s degree in Social Work, Human Services, Counseling, Workforce Development, or related field (or equivalent work experience).
• At least 2 years of experience in case management, workforce development, or human services.
• Knowledge of New York State resources, benefits programs, and community partnerships.
• Strong organizational, interpersonal, and communication skills.
• Ability to manage multiple responsibilities with accuracy and attention to detail.
• Demonstrated empathy, cultural awareness, and problem-solving ability.
