Greenville Nursing & Rehabilitation is seeking an experienced and detail-oriented RN MDS Coordinator to join our clinical leadership team.
This is a great opportunity for a nurse who understands MDS, enjoys working with the interdisciplinary team, and wants to play an important role in resident outcomes, care planning, reimbursement, and overall facility success.
Compensation & Incentives
Registered Nurse (RN): $39.00/hour
$10,000 Sign-On Bonus
Paid bi-weekly over one year
Plus an annual performance evaluation with the potential for a pay increase based on performance.
Schedule
Full-Time
Monday–Friday
Participation in the management on-call rotation is required
Additional hours as needed to support resident care and regulatory requirements

What You'll Do
As our MDS Coordinator, you'll play an important role in keeping our clinical, care planning, and reimbursement processes accurate and on track.
Coordinate and oversee MDS assessments and submissions
Monitor assessment schedules and deadlines
Lead and participate in interdisciplinary care planning
Develop and update individualized resident care plans
Review clinical documentation for accuracy and completeness
Support reimbursement accuracy and quality measures
Maintain compliance with federal, state, and facility requirements
Support survey readiness
Serve as a resource for nursing staff regarding MDS and documentation
What We're Looking For
Current Illinois RN license
Previous MDS experience strongly preferred
Long-term care experience preferred
Knowledge of MDS processes, Medicare, Medicaid, and reimbursement
Strong organizational and time-management skills
Excellent communication and leadership abilities
Ability to work independently and collaboratively
EMR and clinical documentation experience preferred
