As our census grows, we are looking for an experienced and dedicated Social Services Director to join our leadership team and serve as a key advocate for our residents.
Position Overview
Schedule: Full-Time | Monday–Friday, 8:00 AM – 4:30 PM (with rotating weekends)
The Social Services Director plays a vital role in supporting the emotional, social, and psychological well-being of our residents. This position works closely with residents, families, and our interdisciplinary team to ensure each individual receives compassionate, holistic care throughout their stay.

Key Responsibilities
- Conduct comprehensive psychosocial assessments and develop individualized care plans
- Coordinate admissions and discharge planning with residents and families
- Provide emotional support, crisis intervention, and guidance during transitions
- Lead and facilitate care plan and family meetings
- Monitor and address behavioral and mental health needs
- Collaborate with nursing, dietary, activities, and administration teams to ensure holistic care
- Connect residents and families with community resources (financial, legal, counseling, etc.)
- Ensure compliance with CMS, HIPAA, and state regulations
- Advocate for resident rights, dignity, and quality of life
- Provide leadership, guidance, and support to social services staff (if applicable)
Qualifications
- Previous Social Services experience in a Long-Term Care (LTC) setting preferred
- Strong knowledge of CMS, HIPAA, and state regulations
- Experience working with elderly populations, including dementia and Alzheimer’s care
- Excellent communication, documentation, and organizational skills
- Ability to manage sensitive situations with professionalism and compassion
- Strong assessment and care planning skills
- Team-oriented mindset with a passion for resident advocacy
- Leadership experience is a plus
