Position Summary:
Evaluates, plans, organizes, develops, and directs the Physical Therapy (PT) services for assigned patients/residents in accordance with current applicable laws and regulations, and as may be directed by the Director of Rehabilitation and/or Administrator, to assure that the highest degree of quality care can be maintained.
Qualifications:
- Current or pending PT license
- Experience as a therapist in a nursing, hospital, or other related medical facility.
- Training in rehabilitative and restorative therapy practices
- Current CPR/BLS certifications
- Proven organizational and communication skills
- Ability to remain calm and professional in all work-related situations
Responsibilities:
- Plans, develops, organizes, implements, evaluates, and directs the physical therapy for assigned patients/residents in accordance with current laws and regulations that govern nursing care facilities
- Meets with administration, medical and nursing staff, as well as other related departments in planning therapy services
- Maintains treatment records, resident files, and progress notes as required
- Assesses patients’ needs; identifies and initiates appropriate physical therapy interventions
- Observes and evaluates treatment outcomes recommending changes as needed
- Performs assessments and re-assessments as needed on each assigned patient/resident
- Interviews resident to determine interests and types of treatments needed
- Participates in development and implementation of resident assessments (MDS) and care plans, including quarterly reviews
- Assists in transporting resident to and from physical therapy room/area
- Documents patient care in accordance with regulatory, licensing, payer and accrediting requirements
- Educates patient's family/nursing staff/caregivers in follow-through programs
- Maintains equipment and work area in a safe and clean condition
