- Perform initial and ongoing evaluations to assess mobility, strength, balance, and endurance
- Develop and implement individualized, functional-focused treatment plans
- Deliver hands-on interventions to help residents recover from illness, injury, or surgery
- Enhance quality of life by improving functional movement and reducing pain
- Educate residents, families, and staff on safe mobility and fall prevention
- Recommend and train on assistive devices (walkers, wheelchairs, braces, etc.)
- Collaborate with a multidisciplinary care team to ensure holistic, person-centered care
- Complete documentation to ensure continuity of care and compliance
What You Bring
- Active Wisconsin Physical Therapist license
- Strong clinical reasoning and commitment to quality care
- A passion for helping older adults maintain independence and dignity
- Excellent communication, collaboration, and time management skills

- Graduate of an accredited school with a bachelor’s, master’s, or doctoral degree in Physical Therapy.
- License as required by the State of Wisconsin to practice as a Physical Therapist.
