Join our team as a Rehab Director – Physical Therapist or Physical Therapist Assistant
Greenfield Rehab is seeking an experienced and dedicated Physical Therapist (PT) or Physical Therapist Assistant (PTA) to serve as our Rehab Director and lead therapy services in our skilled nursing facility in Galesville, WI.
The Rehabilitation Director is responsible for the overall leadership, coordination, and management of the rehabilitation department within a skilled nursing facility. This role provides clinical and administrative oversight of physical, occupational, and speech therapy services while maintaining a high standard of patient care, regulatory compliance, and operational efficiency. The Rehabilitation Director also functions as a licensed Physical Therapist or Physical Therapist Assistant, providing direct patient care as needed.

Physical Therapy Qualifications
- Graduate of an accredited school with a bachelor’s, master’s, or doctoral degree in Physical Therapy.
- License as required by the State of Wisconsin to practice as a Physical Therapist.
Physical Therapy Assistant Qualifications
- Graduate of an accredited school with an associate degree as a Physical Therapist Assistant.
- Certification or license as required by the State of Wisconsin to practice as a Physical Therapist Assistant.
