GreenField Health and Rehab Center is looking for a dedicated full time Social Worker for our Skilled Nursing Facility!

Social Worker | Role & Responsibilities

  • Conducts an initial psychological assessment and interview with the resident and family to evaluate the appropriateness of placement and identify the need for special services.
  • Interprets the residents’ rights to residents’ families and staff, act as an advocate for residents rights.’
  • Advocating for the resident with personal and social problems, and problems involved with institutionalization.
  • Facilitates needed communication with other disciplines on behalf of the residents, including Medical, Nursing, Dietary, Rehabilitation and Psychiatric Services.
  • Coordinates and monitors needed available services for individual residents to assure optimum level of emotional, physical and psychological well-being and independence based upon educational background.
  • Involves the resident, other disciplines and Administration as appropriate regarding matters such as bed retention, room change, transfer and discharge.
  • Interprets residents’ needs and behaviors and extends professional intervention to all levels of staff suggesting positive approaches, such as alternative to the use of restraints and psychotropic drugs.
  • Initiates and facilitates small group meetings of residents, family and staff directed at a fuller understanding of the institutionalized resident and fuller joint participation in improving the residents’ emotional and physical well-being.
  • Participates in interdisciplinary care plan meetings and team conference.
  • Provides assistance and support to residents’ family members.
  • Arranges for residents and families to meet with Department of Health surveillance staff as necessary.
  • Participates as requested by residents, in the organization and ongoing functioning of the resident councils.
  • Being available at varying schedules, including weekends and evenings, as necessary and requested.
  • Coordinates and facilitates the referral of residents for needed and requested services and outside resources not available in the facility.
  • Organizing bereavement counseling for roommates, families and other affected individuals.
  • Proper documentation of progress notes are to be completed on a timely basis in accordance with government regulations and facility policy.
  • Attendance at inservices, workshops and seminars, where appropriate.
  • Participation and attendance at Social Service meetings and family council meetings.
  • Assistance with inservice training to staff as related to the Social Service Department.
  • Works with the Social Work consultant in order to enhance the department, facility and quality of life of the residents.
  • Health education for residents, families and significant others.
  • Other duties as deemed necessary and appropriate or as may be directed by Administration or the Director of Social Services.
  • Adheres to dress code policy at all times.
  • Adheres to time and attendance policy at all times.
  • Practices safe work habits and follows all safety procedures of the facility.
  • Assures the safety, security, and wellbeing of our residents

Qualifications:

Social Worker | Requirements

  • Bachelor’s Degree in Social Work or in a related field and has experience in a health care setting.
  • Geriatric experience preferred.
