Responsibilities

• Assist patients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support
• Monitor and observe patient health status, including vital signs and overall condition
• Administer medications and perform catheter care following medical instructions and home healthcare regulations
• Support patients with memory care needs such as Alzheimer’s disease and dementia, ensuring safety and comfort
• Document patient information accurately using Electronic Health Record (EHR) systems and maintain confidentiality in accordance with HIPAA standards
• Facilitate patient transfers using proper methods such as Hoyer lifts or patient transfer techniques
• Provide companionship and emotional support, especially for geriatric patients or those receiving hospice or end-of-life care
• Assist with light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe environment

Experience

• Prior experience working in assisted living facilities, nursing homes, or home healthcare settings is highly preferred
• Demonstrated ability to work with individuals with disabilities or developmental disabilities
• Knowledge of medical terminology, medication administration, and medical documentation practices
• Experience providing dementia or Alzheimer’s care, memory care, or hospice support is advantageous
• Familiarity with home health agency operations, EMR/EHR systems, and home health care regulations
• Certification in first aid, basic life support (BLS), CPR, and heavy lifting techniques is desirable
• Proven ability to communicate effectively with clients and team members while maintaining patient confidentiality

Pay: $18.00 - $26.80 per hour
