As an LPN at The Palms at Plantation, you'll play an important role in supporting the health, safety, and well-being of our residents while helping them maintain their independence and quality of life.
What You'll Do- Provide direct nursing and resident care services according to each resident's individualized plan of care.
- Administer medications in accordance with standards of practice, Florida regulations, and community policies and procedures.
- Complete all required medication documentation, reporting, and follow-up.
- Perform resident assessments as required by state regulations and community policy under the direction of nursing leadership.
- Monitor residents for changes in physical, emotional, or psychosocial condition and immediately report concerns to the appropriate supervisor.
- Document relevant observations, clinical procedures, resident care, and communication with residents' responsible parties by the end of each shift.
- Assist with the development and maintenance of individualized resident care plans.
- Communicate effectively with residents, families, physicians, pharmacies, and other members of the care team.
- Provide guidance and support to caregivers and other resident care associates as appropriate.
- Promote resident dignity, independence, safety, and quality of life.
- Follow infection control, safety, emergency preparedness, and resident care procedures.
- Maintain a positive, professional, and team-focused approach to resident care.
- Perform other duties as assigned.
- Current Florida Licensed Practical Nurse (LPN) license in good standing required.
- High school diploma or equivalent (GED) required.
- Current CPR certification required.
- At least 1 year of related nursing experience preferred.
- Senior living, assisted living, memory care, or long-term care experience is a plus.
- Strong communication, organizational, and clinical skills.
- Compassionate and resident-focused approach to care.
- Dependable and able to work effectively both independently and as part of a team.
- Ability to meet the physical requirements of the position, including assisting with resident transfers and repositioning using appropriate techniques and equipment.
