Requirements
Key Responsibilities:
- Review and interpret prescription orders to ensure accuracy and appropriateness of therapy
- Provide patient education regarding medications, including side effects and therapeutic outcomes
- Collaborate with physicians and other healthcare providers to optimize medication therapy
- Monitor patient medication therapies for safety and efficacy
- Maintain accurate records and ensure compliance with all state and federal regulations
- Pharm.D. degree from an accredited pharmacy school
- Valid pharmacist license in the state of practice
- Previous experience in a clinical pharmacy setting preferred
- Strong clinical knowledge and understanding of pharmacotherapy
- Excellent communication and interpersonal skills
- Ability to work effectively in a multidisciplinary team environment
- Commitment to providing high-quality patient care
