In this role, you will provide skilled nursing care, coordinate patient services, and help patients achieve the highest level of independence and quality of life in the comfort of their homes. Position Summary:
The RN Case Manager is responsible for managing a caseload of home health patients by conducting comprehensive assessments, developing individualized plans of care, coordinating interdisciplinary services, and providing skilled nursing care in accordance with physician orders and agency policies.
Responsibilities:
- Provide direct patient care in the home setting by performing comprehensive assessments.
- Develop, implement, and update individualized patient care plans.
- Coordinate care with physicians, therapists, and other healthcare professionals.
- Provide skilled nursing services including assessments, medication management, wound care/treatments, disease education, and patient/caregiver training.
- Monitor patient progress and adjust the plan of care as needed.
- Complete accurate and timely electronic documentation.
- Participate in on-call rotation as assigned.
Qualifications:
- Current Registered Nurse license in the state of Iowa
- Home health experience preferred but not required - we're willing to train the right nurse.
- Strong assessment, communication, and critical thinking skills.
- Ability to work independently while managing multiple priorities.
- Valid Driver's License, reliable transportation and current auto insurance.
