Job description
This is a professional position responsible for a wide range of social services to our residents and their families. The services provided may include: problem solving, community referrals, financial education and guidance.
Essential job functions
- Function as a support person for the patient/family/caregiver on an ongoing basis.
- Provide and maintain a safe environment for the patient.
- Assist patient/family/caregiver and other team members in providing continuity of care.
- Demonstrates an ability to prioritize work responsibilities effectively.
- Ability to develop and maintain good working relationships within the community by maintaining existing resources and developing new ones.
- Deals with conflict situations in a professional manner
Experience:
Minimum of 1-2 years in varied multidisciplinary settings in the health care field.
