General Statement of Responsibilities: The Charge Nurse is responsible for the well being and nursing care of all residents assigned to their care during their shift change, or tour of duty. This includes, but is not limited to: provision of nursing services; and assisting in: carrying out physician’s orders; assessing residents to note any changes in resident’s condition; distribution of medications, chart medication, distribution & count narcotics & maintain narcotic inventory record; provide leadership and guidance to C.N.A. staff, and respond immediately to emergencies.
Bona Fide Occupational Requirement: Possession of current state license as a Licensed Practical Nurse in the Commonwealth of Pennsylvania.
Major Job Duties:
1. Visit each resident as assigned on shift and provide nursing care as ordered.
2. Ensure documentation is complete and accurate prior to end of shift.
3. Notify physician and families of change in resident condition and implement physician’s orders in resident care as instructed.
4. Assist with distribution of medications and chart distribution of medications in resident’s charts according to established standards and regulations.
5. Assist with count of narcotics and maintain accurate record of narcotic inventory/count.
6. Provide information and support to resident and their families as needed.
7. Assist in the transfer of resident’s care and condition as related to nursing services in shift changes; i.e. shift beginning and shift ending.
8. Participate in in-service trainings, staff development programs, and staff meetings.
9. Provide emergency care as appropriate in accordance with physician orders and advanced directives.
10. Overall management and implementation of resident care plan. Document in care plan resident’s progress and change in condition.
11. Provide direction, supervision, and evaluation of nursing assistants for care of residents assigned to them.
12. Provide care in accordance with Federal, State, and Local regulations.
13. Perform other duties as assigned and needed to ensure the quality of comprehensive nursing care and other general care to residents as assigned by the Charge Nurse of Director of Nursing.
Knowledge, Skills, & Abilities In: Best practices in nursing care, quality assurance, state regulations, standards and mandates in long term care; documenting services according to acceptable practice and standards; working with elderly & disables population; work effectively with physicians & other treatment team staff, read and implement physician orders, distribution & inventorying medications; medication side effects; universal precaution/infection control; solve problems & make sound judgments; effective oral written , and interpersonal skills; maintain confidentiality; work and be available for varied/flexible schedule.
Physical Requirements: Must be capable of exerting 50 to 100 lbs. of force occasionally (less than 1/3 of the time) and /or excess of 25-50 lbs. of force frequently and up to 10-25 lbs. constantly; must be able to move objects, equipment, and residents. Must be capable of regularly standing, walking, sitting, stooping, kneeling, bending, and crouching; must use hands to manipulate tools, equipment, or controls; reach with hands and arms, balance, lift and perform medium to maximum transfer assists with elderly residents. Must have visual and hearing acuity sufficient enough to assess resident’s safety and ability.
