Job Summary
We are seeking a compassionate and dedicated Social Worker to join our Hospice team. This job is a part time job and compensation will be $150 per visit made.
Duties
- Conduct assessments to identify clients' needs and develop individualized service plans.
- Provide crisis intervention and support during critical situations.
- Facilitate access to behavioral health services for clients requiring mental health support.
- Maintain accurate documentation of client interactions, progress, and outcomes.
- Advocate for clients' rights and connect them with community resources, including Medicare services.
Experience
Candidates should possess a degree in social work or a related field, along with relevant experience in social services. Preferred skills include:
- Experience in Hospice or working with patients and families.
- Proficiency in crisis management and intervention strategies.
- Understanding of behavioral health issues affecting diverse populations.
