Job Summary: We are seeking a dedicated Licensed Vocational Nurse to join our Hospice team. The ideal candidate will provide quality nursing care to patients in a multiple settings including home, assisted living, board and care, and SNFs. Demonstrating expertise in various medical procedures and patient care.
Duties: - Perform physical examinations and health assessments - Administer medications and injections following established protocols - Provide catheter care and wound dressing changes - Conduct patient and family education on health conditions and treatment plans - Collaborate with healthcare professionals to ensure comprehensive patient care - Maintain accurate patient records using HospiceMD or similar electronic health record systems
Qualifications: - Valid Licensed Vocational Nurse (LVN) license in the state of California.
Proficiency in conducting physical assessments and understanding of human physiology.
Experience in catheter care, ostomy care, pain management, care coordination, wound care.
Ability to triage and handle emergency situations effectively.
Team coordination to establish and follow an effective plan of care.
Physical ability to lift and assist patients as needed.
Knowledge of geriatric patient care practices.
