Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor patients’ vital signs and observe for any changes in health or behavior to ensure safety and well-being
- Administer medications accurately following care plans and HIPAA regulations to protect patient privacy
- Prepare nutritious meals tailored to dietary needs and assist with feeding when necessary
- Operate mobility aids such as Hoyer lifts and provide safe transfer assistance for residents with disabilities or mobility challenges
- Maintain detailed documentation of patient care activities, observations, and progress in compliance with healthcare standards
- Support clients with dementia or Alzheimer’s by implementing specialized memory care techniques and engaging them in meaningful activities
- Perform light housekeeping duties including cleaning, laundry, and maintaining a safe environment for residents
- Collaborate with healthcare professionals to develop and follow personalized care plans that meet each individual’s needs
- Provide emotional support and companionship to promote mental health and social engagement
Qualifications
- Prior experience in home health, assisted living, nursing homes, or group homes is highly preferred
- Certification in CPR and First Aid is required; additional training in hospice or dementia care is a plus
- Knowledge of HIPAA regulations to ensure patient confidentiality at all times
- Experience working with individuals with developmental disabilities, autism, or aging-related conditions such as Alzheimer’s or dementia
- Ability to lift heavy objects safely using equipment like Hoyer lifts when necessary
- Strong observational skills for patient monitoring and recognizing early signs of health issues
- Excellent communication skills to effectively interact with clients, families, and healthcare teams
- Valid driver’s license and reliable transportation are often required for community-based visits
