The purpose of the Unit Clerk is to assist and support the Pleasant View Nursing Home Nursing Department in the processing of clinical information, transcription of orders, scheduling appointments, admissions, and providing the highest practicable quality of care for Pleasant View Nursing Home residents.

Clerical Functions
- Answer telephone calls in a courteous manner. Route the call appropriately.
- Process the resident charge slips and submit them to the Inventory Clerk.
- Prepare the clinical information for Physician rounds. Complete the transcription of physician orders and report discrepancies found in transcribing physician’s orders to the nurse/NCC/Supervisor.
- Assist in scheduling appointments for diagnostic and therapeutic services, physician visits, etc. Communicate schedules and all appointments to the family unless otherwise directed by the resident.
- Prepare the necessary documents for each appointment, file the information in an envelope and label the envelope appropriately.
- Schedule transportation needed for outside appointments. Communicate this schedule to the family as needed.
- Communicates residents’ appointment schedules to nursing staff and reception.
- File clinical information in the clinical records. Input information in the electronic record.
- Record residents’ statistical information on all forms used on charts, and input information in the electronic record.
- Process new physician orders to pharmacy and communicate with pharmacy regarding any changes in M.D. orders. Keep the original sheet of all orders called to the pharmacy for our records.
- Enter new medication or treatment orders in the electronic system and have the nurse verify the order(s).
- Enter new Oxygen orders, notify Oxygen supplier of new orders, monthly billing related to oxygen and cleaning of concentrators.
- Request maintenance request(s) through the Workxhub.
- Scan records into EMR, fax and scan records to providers, ancillary services, etc.
- Greet visitors and answer questions as needed.
- Greet new admissions and escort them to their room.
- Keep nurses’ stations clean.
- Notify the pharmacy when stat or emergency medications are administered.
- Prepare for new admissions; setting up the room; acquiring requested equipment; and other necessary information.
- Notify departments and pharmacies when a resident is transferred to the hospital, or is discharged for any reason.
- Complete Lab Slips as needed.
- Enter Lab orders in electronic systems as needed.
- Notify Inventory Clerk of needed supplies and equipment based on resident needs.
- If unable to arrange outside transportation, unit clerk will provide transportation for resident to scheduled appointments when necessary.
- Understand and follow the Pleasant View and Green County Employee Handbooks.
- Follow facility guidelines and read updates, memos, and educational sessions.
- Fulfill work schedule by arriving at the work station at the start of the shift. Meet attendance standards. Will fill in for open shifts as needed.
- Work cooperatively and collaboratively with other employees, assisting as needed and when requested. Be a positive team member. Refrain from speaking negatively, engaging in gossip and the criticism of fellow employees.
- Be supportive of the nurse, the NCC and/or RN Supervisor and the DON. Communicate in a positive manner. Be an active member of the improvement process by presenting concerns along with suggestions in a timely manner.
- Communicate concerns, questions, frustrations and complaints to the Nurse Care Coordinator/DON in a timely and confidential manner. In the event that this communication is not resolved, communication of this will be presented to the Administrator.
- Understand that performance and conduct reflect positive customer service to internal and external customers.
- Uphold the privacy and confidentiality of resident information at all times.
- Attend all mandatory in-services or completes the make-up session in a timely manner.
- Attend and participates in unit meetings, department meetings and other meetings as assigned.
- Keep personal items, personal cell phones in the employee locker or in the employee’s vehicle. Understand that cell phones are not to be used during paid work time.
- Use the facility computer for resident documentation only. Understand that the computer is facility property and is not to be used for personal use for any reason or method.
- Understand that job performance will be reviewed for a clear understanding of expectations and that job performance that does not meet standards and guidelines may result in disciplinary action up to and including termination.
- Follow established safety regulations in the use of equipment and supplies.
- Assist in maintaining nursing service work areas (i.e. nurses’ stations, medicine preparation rooms, etc.,) in a clean and sanitary manner.
- Participate in and conduct all fire safety and disaster preparedness drills in a safe and professional manner.
- Participate in the implementation, and maintenance of the infection control program for monitoring communicable and/or infectious diseases among the residents and personnel.
- Follow established infection control guidelines when isolation precautions become necessary.
- Report hazardous conditions or equipment.
- Follow established hand washing techniques.
- Setup isolation cart and stock checking.
- Monitor and maintain resident equipment for cleanliness and in proper working order.
- Maintain the confidentiality of all resident care information.
- Treat all residents fairly, and with kindness, dignity, and respect.
- Knock before entering the resident’s room.
- Report immediately resident complaints and/or grievances that indicate the action taken to resolve the complaint and the current status of the complaint immediately.
- Provide data to the Quality Assessment Committee as requested.
- Carry out any other duties as requested by DON, NCC and/or RN Supervisors.

EDUCATION And/or EXPERIENCE
- High school diploma (or equivalent) with vocational training as a ward clerk;
- one year related experience; and
- familiarity with medical terminology required, or any combination of education and experience that provides equivalent knowledge, skills, and abilities.
- Current Certification as a Nursing Assistant (CNA) or Certified Medical Assistant (CMA) highly preferred.
- Valid Drivers License required.
- Pleasant View Nursing Home is a 24 hour facility which requires continuous care for residents, therefore employees must meet attendance standards by arriving to work on time, work all scheduled shifts and extra shifts that are agreed to work and have an overall excellent attendance record.
