Resident Care Coordinator

Green Acres of Cadillac is seeking a Resident Care Coordinator to join our team. This is a rewarding opportunity for an LPN or RN, who is looking for a change of pace and a meaningful career serving seniors.
The ideal candidate is highly organized, an effective communicator, and has a genuine love for working with the elderly. This role plays a vital part in supporting residents, staff, and families while maintaining high-quality care standards within our community.
Key Responsibilities:
- Conduct and oversee level of care assessments
- Coordinate and monitor resident care needs
- Work closely with physicians, hospice providers, and families
- Support, educate, and collaborate with direct care staff
- Order and track medications/scripts as needed
- Complete medication cart audits
- Enter and manage resident care orders
- Communicate effectively with residents, families, and leadership
- Assist with maintaining compliance with care standards, policies, and regulations
- Share on-call responsibilities as part of the leadership team
