What are the responsibilities and job description for the Long Term Services and Supports Coordinator - Bilingual English/Spanish position at Greater Springfield Senior Services, Inc. (GSSSI)?
Full-time 35 hours/week.

Responsible for functioning as a member of the Integrated Care Team (ICT) in an entity contracted with MassHealth as a One Care. To provide an evaluation, independent of the One Care organizations, of the need and eligibility of Members for support and services in the community and act as their advocate as needed on the ICT. To promote the independent living, social model of disability, wellness and recovery models of engaging with disabled individuals.
- Working with the Member, ICT, and informal caregivers (when appropriate) to develop a service plan which addresses the Members' needs and identifies goals to enhance independent living.
- Assessing a Members' functional, health and income status to determine eligibility and appropriateness for community long term care services and to ensure Members have access to public benefits and other community services and programs, assisting with referrals and applications where appropriate.
- With the agreement/approval of the One Care ICT, Initiating, authorizing, and monitoring service plans to effectively assist Members to maintain independent living.
Minimum Requirements:
- Bachelor's Degree in related field preferred but not required
- High School Diploma with two years experience working in human service field with disabled individuals required.
- Requires more than 1 year relevant experience with disabled adults.
- Requires English/Spanish fluency
