We are offering an excellent opportunity for a full time Hospital and Community Liaison with our Community & Crisis department.

Responsibilities

Serves as liaison between Greater Reach Community Services Board and state hospitals for patients who have been civilly admitted to those facilities. Provides advanced level of inpatient hospital management and person-centered discharge planning services to include assessment of patient's needs, referral to community housing resources, referral to Region IV programming, utilization of state hospital discharge protocols, and ability to access state (DAP) and local (Region IV/other) funding streams to maximize patient access to financial resources.

Required Education and Experience

  • Masters degree in behavioral health, Human Services, or related field.
  • Must be registered with the Virginia Department of Health Professions with current LMHP or LMHP-E licensure status.
  • One (1) year position related experience in the emergency services mental health treatment.
  • Valid Drivers License with a point balance of -6 or higher.

  • Any equivalent combination of education, training, and experience which provides the required knowledge and abilities.

Required Knowledge, Skills and Abilities

  • Knowledge of crisis intervention; human personality, growth and development; mental health disabilities; evaluation of mental health disabilities; psychotropic medications and rapid stabilization treatment approaches; substance abuse indicators, progression and crisis; case management; entitlement programs; written, oral and interpersonal communications; interagency services coordination.
  • Demonstrated skill in providing emergency/crisis intervention services; completing psychosocial evaluations; hospital diversion strategies and resources; maintaining casework documentation; providing informal mental health and substance abuse consultation and education services; coordinating services; reporting on program expenditures and outcome measures.
  • Ability to work effectively with all types/levels of persons; exercise sound judgment; demonstrate initiative; maintain confidentiality.
  • Knowledge of the Virginia code which relates to temporary detentions and involuntary commitments. Experience working in a community-based emergency service.
  • This position requires travel to multiple sites located throughout the Greater Reach Community Services Board catchment area and beyond.
  • Competency in the exposure to potentially volatile consumers in community and acute inpatient setting.

Preferred

  • Three (3) years position related experience in the emergency services mental health treatment.
  • One (1) year experience with discharge planning, case management, care coordination, and clinical documentation

Special work conditions

  • Ability to lift 35 pounds (ex: supplies for meetings, items needing to be delivered to the C&CS suites, etc.) Work evenings when required to address board/agency functions. Travel throughout the commonwealth including overnight where applicable.

Fingerprints, State and FBI criminal record reports, drug test, and central registry report will be required upon selection.Our Agency maintains a drug-free workplace.

Annual Salary Range

Grade C9for Licensed Eligible.$60,863.00 - $68,471.00.

Grade C11for Licensed. $69,356.00 - $80,916.00.
