We are offering a full-time DD Intake Coordinator position.

Responsibilities

We are seeking a compassionate and skilled DD Intake Coordinator. The Developmental Disabilities (DD) Intake Coordinator serves as the initial point of contact for individuals and families seeking Developmental Disabilities Services. This position is responsible for coordinating the intake and eligibility determination process, conducting comprehensive assessments, gathering required documentation and connecting individuals with appropriate services and community resources. The Intake Coordinator ensures timely access to services while maintaining compliance with all Department of Behavioral Health and Developmental Services (DBHDS) and all federal, state and agency requirements.

Required Education and Experience

• Bachelor's degree in Human Services, Social Work, Psychology, Special Education, Counseling, Sociology, or a closely related field.
• At least one (1) year experience working with individuals with developmental disabilities or intellectual disabilities.
• Experience conducting assessments, eligibility determinations, intake, or case management preferred.
• Valid driver's license and reliable transportation, as required.

• Ability to manage multiple referrals, deadlines, and priorities simultaneously.
• Excellent problem-solving and critical-thinking abilities.
• Ability to analyze complex documentation and make sound eligibility recommendations.
• Strong attention to detail and accuracy in documentation.
• Ability to work independently while functioning effectively as part of an interdisciplinary team.
• Proficiency with electronic health records, Microsoft Office applications, and other data management systems.
• Commitment to providing compassionate, person-centered, and culturally responsive services and all other duties as assigned including assuming additional caseload responsibilities when needed.

Special Work Conditions

• Work is primarily performed in the office but occasionally conducted in the community, home, school, and healthcare settings.
• Frequent interaction with individuals, families, service providers, and community partners.
• Flexible hours may be required to accommodate individuals and families.
• Local travel may be required.

Fingerprints, State and FBI criminal record reports, drug tests, and central registry reports will be required upon selection. Our Agency maintains a drug-free workplace.

Annual Salary Range

Grade S-C7

$52,576.00 - $59,148.00
