Clinical Care Coordinator
Greater Oregon Behavioral Health, Inc. has an immediate opening for a full-time Clinical Care Coordinator who will provide high quality, integrated, trauma-informed and culturally competent care coordination services to members with complex behavioral health care needs.
Key Responsibilities:
- Develop plans of care to ensure members' needs are met.
- Document and present coherent clinical assessments and case formulations.
- Teach and provide telephonic support for members and those around them.
- Use clinical expertise to provide access to culturally and linguistically appropriate services.
- Coordinate with providers at all levels of care.
Requirements:
- 5 years' experience in mental health and knowledge of current version of the Diagnostic and Statistical Manual of Mental Disorders (DSM) criteria.
- Ability to conduct psychosocial assessments and formulate multiaxial DSM impressions and mental health risk assessments.
- Strong planning and organizational skills, positive attitude, and communication skills.
- Bilingual in Spanish preferred, Master's degree required with major study in social work, psychology, sociology, or a closed allied field.
