Location: Primarily based in White Plains and/or Valley Cottage. This is a mobile role and may require travel to additional service areas in Putnam, Orange, and Sullivan counties, as needed.

Salary Range: $60,000 annually



Summary Description:  Greater Mental Health of New York’s INSET program provides mobile, community-based, and flexible support for individuals with histories of acute distress, frequent hospitalizations, or ineffective past care. Led by peers and grounded in a person-first approach, INSET prioritizes immediate needs, builds lasting social and family connections, and offers care coordination without traditional time or location constraints. The INSET Peer Care Manager combines the values of peer support with care management functions. In this role, the staff member uses lived experience to engage and support participants in their recovery journey, foster resilience, and connect them to natural and community supports. The staff member also provides care management by assisting with resource navigation, service coordination, and follow-up to ensure participants have access to needed medical, behavioral health, and social services. 

 

Responsibilities:                                               

  • Manage a caseload of participants consistent with industry standards, regulatory expectations, and program needs.
  • Provide individual and group peer support using a flexible, trauma-informed, person-first framework.
  • Conduct outreach to engage individuals who may not typically connect with the mental health system.
  • Support participants in developing recovery-oriented goals, using tools such as Wellness Recovery Action Plans (WRAP) and Advanced Directives.
  • Facilitate or co-facilitate networking and support groups as requested.
  • Support transitions from respite, emergency rooms, hospitalizations, or periods of incarceration.
  • Link participants to peer, family, community, and professional supports.
  • Provide care management support, including:
    • Assisting with applications, entitlements, or benefits.
    • Coordinating referrals and care plans with input from participants.
    • Supporting discharge planning and follow-up during/after hospitalizations.
    • Participating in meetings with providers, specialists, or community agencies as appropriate.
  • Document services and care management activities in the Electronic Health Record system.
  • Provide after-hours emergency coverage and crisis intervention as needed.
  • Follow agency policies, professional standards, and regulatory requirements.
  • Other related duties as assigned.

 

Competencies: 

  • A thorough understanding of the process and possibility of recovery for people who are psychiatrically labeled.
  • Awareness of community resources, supports, and systems, collaborating with the participant to make referrals if indicated by the participant 
  • Knowledge of behavioral health diagnoses and symptomatology
  • Strong client engagement skills with individuals across diverse needs.
  • Ability to balance peer support and care management functions effectively.
  • Capacity to work independently while collaborating as part of a multidisciplinary team.
  • Proficiency in Electronic Health Record systems.
  • Commitment to recovery-oriented, person-centered, trauma-informed practice.

 

Key Performance Indicators (KPIs): 

  • Timely and accurate completion of required documentation. 
  • Effective coordination and utilization of community resources. 
  • Responsive and supportive crisis intervention. 
  • Adherence to regulatory standards and agency policies.

 

Requirements: 

  • Certification as a New York Peer Specialist (or eligible for certification within six months of hire)
  • While not required, it is beneficial for the NY Certified Peer Specialist to hold other credentials such as a CRPA, FPA, or YPA credential.
  • 2 years of related work experience (preferably, the candidate will have experience in mobile engagement techniques).
  • Basic computer knowledge. 
  • Valid Driver's License and satisfactory driving record. 
  • Maintain New York State Defensive Driving certification within 30 days of hire. 
  • Must maintain pre-employment clearances by applicable regulatory clearances as required. 
  • Legal eligibility to work in the United States without sponsorship. 
  • Fluency in English (Reading, Writing, Speaking). 

 

 

Special Considerations

  • Weekly full-time schedule may include two evenings per week and weekends when requested.
  • Flexibility with work hours required due to the mobile nature of the program.
  • Location, hours, and reporting structure may change depending on program needs.
     
     
     

Mandated Reporter:

This is a Mandated Reporter position.  The NYS Justice Center legislation defines anyone who has regular and substantial contact with any person with special needs as a “custodian” under the law and, therefore, a mandated reporter.  You can be held liable by both the civil and criminal legal systems for intentionally failing to make a report.
