Summary Description: The Mental Health Professional will work as part of a multidisciplinary team in the Critical Time Intervention (CTI) program serving adults with co-occurring behavioral health and physical health needs who frequent ED/inpatient settings.  The Mental Health Professional completes care management-related tasks.  Additionally, they resolve clinical and rehabilitative needs, when within the scope of practice, that impact the individual’s ability to meet recovery goals.


Responsibilities: 

  • Provide therapeutic interventions for both mental health and substance use disorders.
  • Educate individuals about mental health, treatment, and recovery.
  • Teach skills for coping with specific behavioral health symptoms and stress management.
  • Develop crisis management plans and relapse prevention plans.
  • Identify/recognize early warning signs and rapid intervention strategies.
  • With client’s input, create individualized plans of care.
  • Assist in discharge planning during a client’s hospitalization and provide follow-up care as needed.
  • Consult with team members and other specialists.
  • Participate in meetings with community agencies and service providers.
  • Prepare required reports and documentation.
  • Provide after-hours emergency coverage and crisis intervention as needed.
  • Perform other duties as assigned.

Key Performance Indicators (KPIs): 

  • Effective rapport building and engagement with clients.
  • Effective crisis de-escalation and diversion from ED, CPEP, and/or Inpatient admission.
  • Timely and accurate completion of required screenings and assessments.
  • Meet prescribed timeframes for response and service delivery as outlined by CTI model
  • Timely and thorough documentation of client care.
  • Effective collaboration with the client’s support team, including active involvement in multidisciplinary team meetings.
  • Respond to emergencies and adhere to safety protocols. 
  • Completion and maintenance of all required trainings under the CTI model.
  • Effective collaboration with the support team.

Requirements: 

  • Master’s degree in a relevant field or equivalent and should be pursuing their Limited  Permit or license. For example, social work, psychology, nursing, rehabilitation, education, occupational therapy, physical therapy, recreation or recreation therapy, counseling, community mental health, child and family studies, sociology, speech and hearing or other human services field. 
  • Active and unrestricted limited permit or licensure in a respective field preferred.
  • Two years of relevant experience or equivalent.
  • Valid Driver's License and satisfactory driving record.
  • Maintain New York State Defensive Driving certification within 30 days of hire.
  • Basic computer knowledge: typing, communication tools, scheduling tools and web browsers.
  • Proficiency in Microsoft Office suite and database management systems. 
  • Fluency in English (Reading, Writing, Speaking).
  • Must be and maintain clearance by applicable regulatory clearances as required.
  • Legally eligible to work in the United States without sponsorship. 

Special Considerations

  • This is a full-time, 35-hour per week position with a regular daytime schedule that will include an evening shift and rotating on-call responsibilities. 
  • This position will require availability outside of the standard workday, and as such must be ready and available at times to direct and support staff either by phone or in person if needed during the evenings, weekends and holidays.
  • Hours Per Week, Reporting Structure, and Location are subject to change per program needs


Mandated Reporter:

This is a Mandated Reporter position.  The NYS Justice Center legislation defines anyone who has regular and substantial contact with any person with special needs as a “custodian” under the law and, therefore, a mandated reporter.  You can be held liable by both the civil and criminal legal systems for intentionally failing to make a report.
