What are the responsibilities and job description for the Support Coordinator/Case Manager position at Greater Alabama 310 Board, Inc.?
Entry level Support Coordinator Position - Responsibilities include providing quality services to Medicaid eligible individuals with an intellectual disability and/or developmental disability. Services include case planning, social support, follow-up and monitoring. Must be able to work independently as well as work with a team to benefit the individual being served. Applicant must have a valid driver's license with reliable transportation as travel within the county is required. Required to carry the mandatory vehicle liability insurance as required by the State of AL.
Applicant must be able to pass a background check and drug test.
BA/BS Degree in a Human Services Field is required.
