Position Title: Advanced Practice Provider

Division: OHC

Reports To: Clinic Manager

Direct Reports: N/A

Job Summary

This position, under the direction of a supervisory physician, is responsible for providing high-quality, general medical care and treatment to Oyate Health Center (OHC) patients.

Essential Functions

  • Maintains knowledge and proficiency in medical practices through continuing education and training.
  • Performs physical examinations and preventive health measures within prescribed guidelines and instructions of Physician.
  • Provides medical care to repair minor wounds and injuries.
  • Administer vaccinations.
  • Collects appropriate sample for medical laboratory testing.
  • Orders, interprets, and evaluates diagnostic tests to identify and assess patient's clinical problems and health care needs.
  • Documents physical findings and formulates a plan and prognosis based on the patient's condition.
  • Discusses case with Physician and other health professionals to prepare a comprehensive patient care plan as appropriate.
  • Prescribes or recommends drugs or other forms of treatment such as physical therapy, inhalation therapy, or related therapeutic procedures.
  • Refers patients to Specialist for consultation or to specialized health resources for treatment appropriately.
  • Ensures accurate information is documented within the patient’s electronic health record.
  • Leads care pod to enhance proactive and preventive care for empaneled patients.
  • Incorporates disease management best practice guidelines in medical management practices.
  • Communicates abnormal and unexpected test results directly with patients in a timely manner.
  • Incorporates/ honors cultural specific patient wishes and practices into care plan.
  • Must comply with federal laws and regulations as required by the Health Insurance Portability and Accountability Act (HIPPA).
  • Performs related duties.

Knowledge Required

  • Knowledge of diagnostic, preventive, and therapeutic medical care.
  • Knowledge of the scientific process including assessment, planning, implementation, and evaluation of complex and subtle patient needs and interventions.
  • Knowledge of exposure control including bloodborne pathogens and respiratory protection protocols.
  • Knowledge of State of South Dakota Immunization Program policies and procedures.
  • Knowledge of diversity and cultural difference of Native American populations, and the ability to appreciate cultural differences and their effect on healthcare delivery.
  • Ability to work with personal computer and utilize a variety of software applications, including database and office software systems.
  • Ability to communicate clearly and effectively, both orally and in writing, using tact and sensitivity.
  • Ability to collect, organize, record, and proficiently communicate data relevant to primary health assessments.
  • Ability to prioritize and shift priorities in a changing environment.
  • Ability to organize the multiple demands of the job.
  • Skill in identifying factors that adversely impact health status and are not susceptible to treatment by accepted methods.
  • Skill in prioritizing and planning.
  • Skill in interpersonal relations.
  • Skill in use of personal computer and a variety of job related software applications.

Physical Requirements

This position involves primarily sedentary work with periods of standing and walking. The role requires frequent use of hands and fingers for documentation, data entry, and clinical procedures. The employee must be able to communicate effectively, hear and respond to patient needs, and maintain visual acuity for patient assessment and documentation. Occasional lifting of up to 25 pounds may be required. The position involves exposure to clinical environments and requires adherence to infection control and safety protocols, including the use of personal protective equipment.

Education/Experience

  • Completion of an accredited Physician Assistant Program or Certified Nurse Practitioner.
  • One year of clinical experience practicing as a mid-level provider, preferred.
  • Must have a permanent, full, and unrestricted license as an Advanced Practice Provider.
  • Must obtain and maintain medical staff clinical privileges, including any licensure requirements.
  • Basic Life Support (BLS) certification is required or must be obtained within ninety (90) days of employment.
  • ACLS certification and PALS certification preferred.
