Job Objective(s):
Responsible for the coordination of services available to residents, including health services and housekeeping. Assists with resident’s adjustment to the facility. Communicate with residents and family during transitions.
Qualifications:
- Strong desire to work with elderly people in need of services. Understanding the aging process.
- Excellent communication skills.
- Ability to work as a team member.
- Strong knowledge of community resources and care options.
- Ability to assist tenants and families in making difficult decisions and facilitating transitions.
- Able to read, write, spell, do basic math, speak and understand English.
Job Responsibilities and Duties:
- Coordinate the continuum of personal and health services for residents.
- Meet weekly with Executive Director
- Conduct admission interviews and meet with the Admission Committee to ensure appropriate moves, write up interview notes to distribute to appropriate staff.
- Market programs to present and prospective residents who will require services to live independently and to make appropriate referrals for services.
- Welcome and orient new residents upon move-in with service information.
- Distribute welcome notices to apartments or cubbies for new residents’ neighbors. Make sure KARDEX information is completed for each new resident and updated as needed. Assist with the welcome social.
- Provide monthly reports to the Executive Director regarding supportive service activity and changes.
- Contact families in the event of resident health or service concerns, keeping the Executive Director and nurse informed of the situation. Schedule and attend family conferences.
- Assist with information about community resources to residents and family (transportation, external services).
- Answer emergency calls during the day as needed. Assist with completion of daily “I’m Okay” checks.
- Attend weekly staff meetings and monthly resident meetings. Attend educational conferences as time permits.
- Other duties as assigned by management.
Must be able to continuously perform tasks such as, but not limited to, those listed below:
- Move, push, pull and/or lift objects weighing 10 to 25 pounds.
- Use hands and arms for simple grasping, pushing and pulling
- Perform tasks requiring actions of muscles or groups of muscles to walk, stoop, bend, lift, and carry objects
- Walk and/or stand for prolonged periods of time
- Speak clearly and make self understood in face-to-face interactions
- See objects closely to read directions, instructions, etc.
- Hear normal sounds and voices with some background noise to receive verbal instructions, answer telephones, communicate with others, etc.
Environment:
Works indoors in a climate-controlled environment majority of the time.
Working Hours:
Typically Monday through Friday 8:30am – 5:00pm. The work schedule and hours may vary from week to week. Every other weekend required.
