Speech-Language Pathologist (SLP-CCC) – Skilled Nursing Facility | Hornell, NY

Key Responsibilities
- Conduct comprehensive speech, language, cognitive, and swallowing evaluations for skilled nursing facility residents
- Develop and implement individualized treatment plans based on assessment findings and resident goals
- Provide evidence-based speech therapy, language therapy, cognitive therapy, and dysphagia treatment
- Coordinate audiometric screenings and referrals to appropriate specialists as needed
- Collaborate with interdisciplinary care teams including physicians, nurses, occupational therapists, and physical therapists
- Maintain accurate clinical documentation, progress notes, and Medicare-compliant records
- Participate in care planning meetings and interdisciplinary team conferences
- Communicate regularly with residents' families regarding treatment progress and goals
- Provide in-service education and training to nursing staff on communication and swallowing safety
- Ensure compliance with state regulations, ASHA standards, and facility policies
- Contribute to quality improvement initiatives and clinical program development
Required Qualifications
- Master's Degree (MA or MS) in Speech-Language Pathology from an accredited program
- Current New York State Speech-Language Pathologist license in good standing
- Certificate of Clinical Competence (CCC-SLP) from the American Speech-Language-Hearing Association (ASHA)
- Strong clinical assessment and treatment planning skills
- Excellent verbal and written communication abilities
- Experience with Medicare documentation and skilled nursing facility regulations preferred
- Proficiency with electronic medical records (EMR) systems
- Team-oriented mindset with collaborative approach to patient care
