Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor patient health by observing vital signs and reporting changes promptly using Electronic Health Record (EHR) systems
- Administer medications accurately following medical documentation and home healthcare regulations
- Support patients with memory care needs such as Alzheimer’s and dementia, implementing specialized care plans
- Use patient transfer methods like Hoyer lifts safely to ensure smooth mobility assistance
- Maintain patient confidentiality in compliance with HIPAA standards while documenting medical records and notes
- Provide emotional support and effective communication in Spanish to enhance client comfort and understanding
Requirements
- Fluent in Spanish with excellent verbal and written communication skills
- Prior experience working with geriatric patients, individuals with developmental disabilities, or in hospice/end-of-life care settings
- Knowledge of medical terminology, catheter care, wound management, and basic life support (BLS) including CPR certification
- Familiarity with home health care assistance, assisted living environments, nursing homes, or home & community care experience
- Ability to perform heavy lifting safely and use patient transfer equipment such as Hoyer lifts
- Understanding of home health agency operations, home health regulations, and medical documentation procedures
- Compassionate attitude with strong patient observation skills and a commitment to providing high-quality patient personal care assistance
