Granville Vance Public Health is currently accepting applications for one (1) Full-time Social Worker-II to work in our Public Health Care Manager -CMHRP in Granville and Vance counties.
THE PRIMARY PURPOSE: General Statement of Duties: The Care Management for High-Risk Pregnancy (CMHRP) Social Worker II is a health professional who assists pregnant clients in accessing health care and support services to reduce infant mortality through improved pregnancy outcomes; ensures the provision of comprehensive preventive health services to pregnant women and their infants; and assist pregnant women to meet other priority needs that affect their well-being and that of their families. Care management services are offered through outreach, conducting assessments, service provision, coordination, referral, follow-up/ monitoring and advocacy. Care Managers work closely with the patient, her support system, medical providers as well as other community service providers. Contact is made with the patient by home visits, community encounters as well as at the pregnancy medical home practice visits. Pregnancy Care Management services are provided for pregnant Medicaid recipients who are determined to be at risk for poor birth outcomes.
MINIMUM QUALIFICATIONS: (BSW, BA in social work, or BS in social work) or (MSW, MA in social work, or MS in social work) from a Council on Social Work Education accredited social work degree program and one year of directly related experience; bachelor’s degree from an appropriately accredited institution and two years directly related experience; or equivalent combination of education and experience.
