Duties
- Perform comprehensive patient assessments, including vital signs, physical examinations, and medical history documentation using EMR (Electronic Medical Records) systems such as Epic or Cerner.
- Administer medications and injections accurately following prescribed protocols, including IV infusions and medication management for acute pain or chronic conditions.
- Assist with medical procedures like catheterization, suturing, tube feeding, and specimen collection while maintaining aseptic techniques.
- Monitor patients’ conditions closely, providing triage support in urgent or emergency situations, including airway management and ventilator care when necessary.
- Collaborate with physicians and interdisciplinary teams to develop and implement personalized care plans for diverse populations such as geriatrics, pediatrics, neonatal patients, or individuals with developmental disabilities or dementia.
- Document all patient interactions meticulously in compliance with HIPAA regulations and ICD coding standards like ICD-10 or ICD-9 for accurate medical records.
- Support discharge planning and case management efforts to ensure smooth transitions of care across settings such as outpatient clinics or home & community care environments.
Requirements
- Valid state licensure as a Licensed Practical Nurse (LPN) with current certification to practice in your jurisdiction.
- Proven experience in critical care settings such as ICU or Level I Trauma Centers is highly desirable; experience in emergency medicine or hospital medicine is a plus.
- Strong knowledge of physiology, anatomy, pharmacology, and medical terminology to perform accurate patient assessments and diagnostics.
- Proficiency with EMR/EHR systems like Epic, Cerner, Athenahealth, or eClinicalWorks for efficient documentation and communication.
- Hands-on experience with IV insertion, phlebotomy, medication administration—including injections—and sterile processing techniques.
- Familiarity with specialized care areas such as Alzheimer’s/dementia/memory care units, hospice care, infectious disease management, or post-acute care environments is advantageous.
- Ability to work effectively with diverse populations including children (working with children), seniors (geriatrics), individuals with disabilities or behavioral health needs.
- Excellent communication skills coupled with a compassionate approach to patient-centered care; ability to work collaboratively within multidisciplinary teams.
