Job Overview
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team at a long-term care and rehab facility. In this role, you will provide essential nursing care to residents, supporting their health and well-being through direct patient care, medication administration, and health monitoring. The ideal candidate will be committed to delivering high-quality care in a supportive environment, ensuring residents' comfort and safety on a daily basis.
Responsibilities
- Provide direct nursing care to residents in a long-term care and rehabilitation setting, ensuring their physical and emotional needs are met.
- Administer medications and treatments accurately according to physician orders and facility protocols.
- Monitor residents’ health status by observing, recording, and reporting changes in condition.
- Assist with activities of daily living such as bathing, dressing, grooming, and mobility support.
- Collaborate with interdisciplinary team members to develop and implement individualized care plans.
- Maintain accurate documentation of resident care, treatments, and progress notes in compliance with regulatory standards.
- Ensure a safe environment by adhering to infection control procedures and safety protocols.
- Educate residents and their families about health conditions, medications, and post-discharge care instructions.
Skills
- Valid licensure as a Licensed Practical Nurse (LPN) with current state certification.
- Experience working in long-term care or rehabilitation facilities is preferred.
- Strong clinical skills with the ability to perform basic nursing procedures confidently.
- Excellent communication skills for effective interaction with residents, families, and team members.
