TITLE: Part-time Bilingual Case Worker
DEPARTMENT: Senior Services
PROGRAM: BEST/NORC - 72 Columbia St., New York, NY 10002
REPORTS TO: Program Director
LOCATION: New York, NY (Lower East Side)
SALARY: $23 per hour; 21 hours per week
SCHEDULE: Monday–Friday, 8:30am – 4:30pm. Weekends as needed.
DATE: October 2025
JOB SUMMARY: Supports Case Assistance and Case Management of the BEST program by coordinating activities and tasks that ensures efficient and effective delivery of services to the program participants. The case worker will provide the following services to meet program participant’s health and psychosocial needs, with a focus on homebound seniors at risk of social isolation: (1) intake assessment and updates, (2) case planning and coordination, (3) evaluation, (4) advocacy for services to participants

Education, Experience And Skill Requirement
- BA in social work, psychology, or a related field preferred or Associate Degree in Human Services
- Excellent communication, analytical and problem-solving skills
- Bilingual English/Spanish
- Experience in the delivery of social services to seniors
- Strong computer skills: Excel, Word & Data Entry
- Provides case assistance and case management to participants, documents intervention in client files, tracks services in worker logs and enter data into funder database (DFTA PeerPlace)
- Identify and provide Norc Assessment to homebound seniors residing in the Baruch Housing, with a focus on those who are at risk of social isolation.
- Conduct initial interview and complete intake assessment that identifies participant’s needs. In collaboration with the client, formulate goals and objectives, as well as appropriate time-line for completion.
- Assist participants in obtaining services and entitlements including Medicaid, food stamps, SCRIE, home care referrals, and others.
- Complete enrollment of all NDA clients which includes assessment, follow up notes, and conducting outcome. Enter data into funder database (DYCD Connect). It will be provided at all three sites for enrollment of NDA clients.
- Assist providing GMHI screening to support increasing Baruch community’s mental health awareness.
- Assists with coordination of program events as needed
- Conducted home visits with our home bound participants as needed
- Provided translation services for clients for our nurse services and counseling services
- Assists in the Food Distributions/Food Pantry as needed
- Connect seniors to GSS Senior Center/NORC/Essex Crossing activities
- Other special projects or miscellaneous program duties as needed
