POSITION DESCRIPTION
Responsible for care coordination for all residents ensuring that quality care, supervision and excellent customer service are provided. Maintains service plans that are updated, accurate, implemented and in compliance.
QUALIFICATIONS
- One year of expierence as caregiver (Resident Assitant) and one year of expierence as a (Med Tech) preferably in a large busy RCFE setting.
- EMAR expierence prefered
- Knowledge of Title 22, Division 6, Chapter 8 Residential Care prefered
- HS diploma or equivalent
- Ablity to read, write and communicate effectively in English
- Must be a self starter and able to work independently
- Flexiblity with schedules and hours including weekends, holidays, PM and Noc shift as needed
- CPR and First Aid
RESPONSIBILITIES
- Will complete resident needs and service plans, appriasls at a minimum of quarterly, or if there is a change in condition
- Will follow up, communicate and coordinate care and services with third party providers and referal sources. This includes but is not limited to Hospice, Home Health, DME equipment, pharmacies, physcians offices, labratory, skilled nursing facilities, and hospitals.
- Incident reports, follow up and implementation of new intervnetions
- Document and resolve resident and family grievances and customer service concerns
- Conduct ADL care audits and med room audits as needed.
- Will monitors supplies, order and stock building as appropriate to mantain sufficent PAR levels.
- Will be a leader in working with RAs and MTs to problem solve on the floor.
- Staffing and scheduling; May be required to pick up shifts as necessary.
- Assist and coordinate social services concerns for residents as they arise
- Will partner wtih lead Med Tech to ensure timely and accurate medication delivery with the pharmacies.
- Will assist with maintaining, updating and monitoring shower schedules
- Will assit with training and onboarding of new care staff.
