What are the responsibilities and job description for the Mental Health Co-Response Case Manager position at GRAND Mental Health?
Care Coordinator for Release to Recovery Delaware County Jail Program. Works directly with DCJ treatment team in the jail, along with jail staff. Goal is to create successful pathways after jail release to out patient, substance abuse treatment, housing, job help. R to R with track and record success and failures to constantly support and improve client care.
Qualifications
Bachelors Degree and/or ability to obtain Case Management II Certificate.
Must have a valid Oklahoma Drivers license.
Must have reliable transportation and proof of insurance.
An expectation of core competencies in relation to individuals with co-occurring disorders.
