Duties
- Perform comprehensive patient assessments including vital signs, physical examinations, and physiological evaluations to inform care plans.
- Administer medications and injections accurately while following strict protocols and maintaining detailed medical documentation.
- Assist with invasive procedures such as IV infusions, catheterizations, tube feedings, and sterile dressing changes using aseptic techniques.
- Support emergency and acute care situations by managing airway patency, ventilator support, and participating in triage processes for urgent cases.
- Document all patient interactions thoroughly within electronic health record (EHR) systems like Epic or Cerner, ensuring compliance with HIPAA regulations.
- Collaborate with multidisciplinary teams including physicians, case managers, and specialists in areas such as cardiology, neurology, pediatrics, geriatrics, and behavioral health.
- Provide patient education on medication management, disease processes like dementia or infectious diseases, and post-discharge care planning to empower patients and families.
Qualifications
- Valid state licensure as a Licensed Practical Nurse (LPN) with current certification.
- Proven experience in various healthcare environments such as hospitals (including ICU or trauma centers), nursing homes, assisted living facilities, or home care settings.
- Strong knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/10), and pharmacology principles.
- Hands-on experience with EMR/EHR systems like Epic, Cerner, Athenahealth or eClinicalWorks for accurate medical documentation.
- Skilled in performing venipuncture (venipuncture), phlebotomy, specimen collection/processing, and administering injections with precision.
- Ability to handle complex patient cases involving pain management, infection control measures, hospice care or behavioral health needs.
- Excellent communication skills for effective patient assessment and working with people with disabilities or developmental challenges.
