Responsibilities

This position supports Grand Itasca Clinic & Hospital's Patient Safety and Regulatory programs by facilitating the management of patient safety events and coordinating the Safety Always program as well as having responsibility for regulatory compliance and accreditation activities related to The Joint Commission (TJC) Standards, the Centers for Medicare and Medicaid services (CMS) Conditions of Participation and the Minnesota Department of Health (MDH)


Grand Itasca Patient Safety Specialist will review & triage daily events, tasking as appropriate, monitoring progress of issue closure, and escalating concerns when appropriate. Will assist with communicating to providers and leadership regarding patient safety events, partner with the leaders to ensure standard work processes are followed and appropriate teams are aware of status of patient safety events. Assists with maintaining appropriate documentation about status of patient safety events following standard workflows, creating event timelines, and conducts EMR chart reviews to assist with event investigations. The Patient Safety Specialist will facilitate event investigations, structured reviews and Root Cause Analysis. Facilitates Safety Always program, coordinating training and safety captains, participates in the Fairview Health System patient safety committees and attends daily safety huddle.


Grand Itasca Regulatory Lead facilitates effective implementation of Grand Itasca Clinic & Hospital's evaluation process to assess the level of compliance to accreditation standards, regulatory requirements and other mandatory regulations (conduct mock JC/CMS surveys). Manages the Accreditation Management Program (AMP) to include: (the AMP is a computerized tool to conduct tracers for JC/CMS preparedness). Designs and directs the effective implementation of formal reporting processes through AMP to assure that leaders have the information they need to make changes and improvements in performance as they relate to accreditation and regulatory compliance.. Assists in the effective implementation of the process to assure that the organization's external surveys (MDH, JC certification) are coordinated from initial application through scheduling, arrangements, hospitality, document review, and follow-through on all related areas. Provides ongoing review, evaluation and interpretation of standards and requirements when there are changes, questions, and new regulations. Manages the Joint Commission Chapter Leads Committee by developing monthly meeting agendas, providing tracer results, educating on new and updated standards and facilitating compliance activities, and participate in regulatory visits and attending Fairview Health System regulatory team meetings.


Qualifications

Minimum Education and Experience:

  • Bachelor's Degree with 5 years of clinical experience in healthcare-related field required.
  • Knowledge of Microsoft suite products and data base applications.
  • Ability to communicate effectively with employees at all levels of the organization, both verbally and in writing.
  • Demonstrated ability to follow and adhere to Grand Itasca Clinic and Hospital policies, vision, mission and values.

Preferred Experience:

  • Knowledge of quality improvement methods, project management experience/facilitation.
  • Experience in data development, analysis and problem-solving.
