JOB SUMMARY

Under relevant clinical and general supervision required by statute or rule, this position serves as a member of a two-person crisis co-response team, responding in person to mental or behavioral health or substance abuse crises alongside other emergency services to provide evaluation, Colorado Crisis Assessment, and recommendations on the most appropriate care plan, including assessing individual risk for suicide and other safety related concerns, and determining the need for resource referrals or involuntary placement. The incumbent regularly completes in-person or phone outreach and follow-up to support an individual's post-crisis care. Post-incident follow-up could also be after other 9-1-1 calls, such as domestic violence, fatalities, major incidents, or traumatic experiences.

ESSENTIAL JOB FUNCTIONS

● Adheres to the Mission, Vision and Values of Grand County Emergency Medical Services.

● Treats all persons encountered in the line of duty with respect, dignity, and empathy.

● Serves as a liaison with other County departments and staff, outside agencies, and the general public.

● Fields and responds to internal and external referrals (from EMS, Fire, Law Enforcement, and Community Partners) to enroll appropriate clients into ongoing preventative care or integrated health services

● Collaborates with Paramedics / Integrated Health teams by conducting social and mental health assessments for enrolled clients and assisting with scheduled follow-up visits

● Provides ongoing resource navigation and follow-up care in the days and weeks following an initial client contact or 9-1-1 response to reduce reliance on emergency services

● Prepares clear and concise written correspondence, reports, and other materials requiring independent judgement as to content, accuracy, and completeness.

● Maintains and manages private, sensitive, and confidential information, which may include protected health information and/or protected criminal justice information.

● Administers and maintains grants. May assist with grant identification and application.

● Attends and participates in meetings and training as assigned. May conduct meetings or training as needed.

● Coordinates care for clients' ongoing bio-psycho-social needs by partnering with local and regional health providers to ensure long-term crisis resolution and stability

● Reports outside of normal working hours to major emergencies, incidents, or catastrophes as directed by the EMS Chief to support public safety operations.

● Performs other related job duties as assigned.

Pay: $68,688.13 - $102,354.12 per year

Education:

  • Bachelor's (Preferred)

License/Certification:

  • LMFT (Preferred)
