Join our client's care team as a Home Health Physical Therapy Assistant, providing compassionate support to patients in their homes. You will help manage and deliver patient care plans while working closely with a team dedicated to outstanding care.
Key Responsibilities:
Schedule: Monday-Friday, 8am-5pm *no on-call or weekends
Key Responsibilities:
- Collaborate to provide management and delivery of patient care plans
- Support patients in Lebanon, PA and surrounding areas
- Meet critical deadlines for charting
- Self-manage time and schedule effectively Travel
- Ability to spend approximately 20% of the day driving to/from patient locations
