Job Summary

Jail In-Reach and Re-Entry Support Specialist

This position will support the re-entry of Men over the age 25 coming out of the Fulton County jail. Will do jail in-reach for individuals that screen positive on the Correctional Mental Health Screen. Will develop individualized release plans and ensure appropriate and timely re-entry support, including Behavioral health, medical, housing and other supports. Will support men up to 1 year, post-release, with continuous support.

QUALIFICATIONS:

• High school diploma or GED is required
• Current certification as Peer Specialist (CPS) through the Georgia DBHDD/Georgia Mental Health Consumer Network is required
• Two years of experience with Serious and Persistent Mental Illness (SPMI) and/or addictive disease is required
• Current American Heart Association Basic Life Support (BLS) Health Care Provider Card is required
• CARES and WRAP certifications is preferred
• Bachelor's degree is preferred
