Position Summary
We are seeking a dedicated and organized Case Coordinator to join our Home Care team. The ideal candidate will be bilingual in English and Spanish, possess strong communication and coordination skills, and have experience working in home care or a related healthcare field. This role is responsible for managing client cases, scheduling aides, maintaining compliance, and ensuring quality client care and satisfaction.
Key Responsibilities
- Coordinate and manage client care schedules in accordance with the plan of care.
- Communicate effectively with clients, family members, caregivers, and healthcare professionals.
- Maintain accurate and up-to-date client records in the agency’s system.
- Ensure timely follow-up on client needs, caregiver attendance, and service quality.
- Handle inquiries from new and existing clients and assist with service coordination.
- Support compliance with state regulations and agency policies.
- Assist with staff assignments, timesheets, and documentation review.
- Participate in team meetings and contribute to continuous improvement efforts.
Qualifications
- Education: Minimum Graduate (Bachelor’s degree preferred).
- Experience: At least 2 years of relevant experience in home care, healthcare coordination, or related field preferred (first priority).
- Language Skills: Must be Bilingual – English and Spanish (Required).
- Computer Skills: Proficient in Microsoft Office and home care scheduling software.
- Other Skills: Excellent communication, multitasking, and organizational abilities.
Work Schedule
- Monday to Friday, [9:00 AM – 5:00 PM]
- Occasional after-hours coordination as needed.
Job Type: Full-time
Pay: $16.00 - $18.00 per hour
Expected hours: 40 per week
