This will be a Part-time Weekend Only Position

Position Summary:

Responsible for evaluation, plan of care, treatment, re-evaluation, discharge, documentation, and modification of all client programs. Administers physical therapy in compliance with Federal, State, local and Health at Home standards, policies, and guidelines.

Job duties: Administers physical therapy in compliance with Federal, State, local and Health at Home standards, policies, and guidelines ensuring quality care and service to Health at Home clients.
Interacts with health professionals, clients, families, and support groups to ensure the maximum rehabilitation, while serving as a client advocate at all times.
Evaluates clients by administering muscle, neurological, gait, and functional tests.
Establishes treatment goals based on the evaluation of the client in conjunction with the client's needs.
Develops treatment programs and provides ongoing assessments and modification as changes in client's condition occur.
Prepares, transmits, and maintains timely and accurate records to include the initial evaluation, progress notes, discharge summary, and attendance records in accordance with agency policy and that support charges for services provided.
Promptly reports incidents, accidents, changes of condition, abuse, concerns to the assigned staff nurse or the Director of Health at Home.
Complies with OSHA standards and ensures the safety of the client in their care at all times.
Attends scheduled in services, training, and staff meetings.

Qualifications:

Bachelor's degree from an accredited college in Physical Therapy (Master's degree preferred). Current Ohio Physical Therapy License. CPR certification. Valid drivers' license and safe driving record.

At least one year physical therapy experience in home health care setting preferred.
