Registered Nurse (RN) – Home Care Supervisor (PRN)

Graceful Pathways LLC is a licensed in-home care agency currently serving seniors, veterans, and adults with disabilities in Sumter and Horry Counties, SC, with plans to grow our service area throughout the state. We're looking for a PRN Registered Nurse to lead the clinical/supervisory side of our care delivery — ensuring every client's care plan meets South Carolina DHEC/DPH and Medicaid compliance standards.
Key Responsibilities:
- Conduct initial client nursing assessments and develop individualized plans of care
- Perform supervisory visits within 30 days of the start of care for new clients, and no less than every 62 days thereafter
- Monitor, train, and evaluate the performance of home care aides/caregivers
- Maintain documentation meeting DPH licensing and SC Healthy Connections Medicaid standards
- Serve as backup/alternate RN Administrator during the primary Administrator's absence, assuming:
- Oversight of non-medical care services
- Review of care plans for appropriateness within non-medical scope
- Caregiver training support (ADLs, safety, documentation)
- Incident review and quality assurance
- Ensuring ongoing compliance with non-medical in-home care plan (IHCP) standards
- Complete Graceful Pathways' administrator-continuity orientation prior to serving in a backup capacity
Qualifications:
- Active, unencumbered South Carolina RN license
- Minimum 1 year nursing experience, including some supervisory or administrative experience, preferably in home care or community health
- Working knowledge of SC in-home care licensing regulations and Medicaid HCBS documentation standards
- Reliable transportation for travel between Sumter and Horry Counties
- Must pass SLED criminal background check and TB screening
Schedule: PRN (as-needed)
Pay:
- $45–$50 per supervisory/assessment visit mileage reimbursement
- $75–$100/day administrator-coverage stipend for periods actively serving as backup RN Administrator
