Graceful Path Youth Home, Inc. is an emerging residential program dedicated to providing structured, trauma-informed care for adolescent females ages 13–17. Our focus is on creating a stable, supportive environment that promotes growth, accountability, and healing through individualized service planning and consistent staff engagement.
We are seeking a Qualified Professional (QP) to support program development, clinical oversight, and compliance readiness as we move toward full operational status. The ideal candidate will bring experience, professionalism, and strong knowledge of North Carolina residential service standards.
Position Summary
The Qualified Professional (QP)serves as the primary clinical and compliance authority for the program, responsible for ensuring all services meet state, Medicaid, and LME/MCO requirements. This includes oversight of Person-Centered Plans, service delivery, staff supervision, and audit readiness. This position requires a minimum of 10 in-person hours per week for on-site engagement with staff, client service review, and compliance monitoring.
This Role Is NOT Entry-Level
- This position requires prior experience functioning in a Qualified Professional (QP), case management, or clinical oversight role within a behavioral health setting.
Candidates must have direct experience with:
- Person-Centered Planning (PCPs)
- Documentation review and approval
- Medicaid and state compliance standards

Key Responsibilities
Clinical Oversight & Compliance
- Own and ensure completion, review, and compliance of all Person-Centered Plans (PCPs)
- Review and sign treatment and service plans, progress notes, and related documentation for compliance with state and LME/MCO standards.
- Provide weekly supervision to direct care and support staff, documenting all supervisory contacts.
- Monitor adherence to client rights, confidentiality, and safety standards.
- Participate in internal audits, training, and program quality reviews.
- Maintain familiarity with 10A NCAC 27G .5600C and related regulations governing 24-hour residential services.
Client & Program Support
- Oversee assessments and ensure individualized plans are timely, goal-oriented, and clinically appropriate.
- Participate in interdisciplinary meetings and ensure effective communication with guardians, DSS, and service providers.
- Review and update service plans following behavioral incidents or significant changes in client status.
- Provide clinical direction during crisis situations and document required follow-up actions.
Crisis Response
- Respond promptly to crisis calls:
- Phone or virtual contact within 1 hour
- In-person or clinical follow-up within 2 hours if required
- Ensure written documentation within 24 hours and treatment-plan revisions within 72 hours as needed.
Training & Leadership
- Support staff development by modeling trauma-informed and evidence-based care practices.
- Provide training and guidance on documentation, crisis response, and behavior support procedures.
Minimum Qualifications (Must Meet ALL Requirements):
- Must be able to independently develop, review, and approve Person-Centered Plans (PCPs) in accordance with NC Medicaid standards
- Must meet Qualified Professional (QP) criteria under NC 10A NCAC 27G .0104
- Bachelor’s degree or higher in a human services field (Psychology, Social Work, Counseling, etc.)
- Minimum 2 years of direct experience working with MH/DD/SAS populations, including:
- Developing or overseeing Person-Centered Plans (PCPs)
- Participating in or leading Child and Family Team (CFT) meetings
- Coordinating care and monitoring service delivery
- Experience with Medicaid documentation, service authorizations, and compliance standards
- Experience working with adolescents or residential settings strongly preferred
Preferred
- Experience in a residential or group home setting.
- Familiarity with Partners Health Management, IRIS reporting, or electronic health record systems (e.g., ECP).
- Current certifications in CPR, First Aid, and NCI (Nonviolent Crisis Intervention).
Schedule & Environment
- Hybrid schedule: combination of on-site and remote administrative hours.
- Must be available for after-hours crisis consultation when needed.
Compensation
- 1099 independent contractor role
- Compensation based on experience, scope, and weekly on-site requirements
- Structured monthly or hourly rate to be discussed during interview
Pay: $30.00 - $40.00 per hour
