About the Role:
Graceful Legacy Hospice in Fredericksburg, VA is looking for a compassionate and dedicated Hospice RN to join our care team. Responsibilities:
- Conduct comprehensive patient assessments and develop individualized plans of care for hospice patients
- Manage pain and symptom control to maximize patient comfort and quality of life
- Coordinate care with interdisciplinary team members including physicians, social workers, and chaplains
- Provide education and emotional support to patients and families regarding the hospice process
- Perform skilled nursing visits in patient homes, assisted living facilities, or care settings throughout the Fredericksburg, VA area
- Accurately document patient care, medication administration, and clinical notes in compliance with hospice regulations
- Respond to after-hours calls and provide on-call nursing support as needed
Requirements:
- Active RN license in the state of Virginia
- Minimum 1-2 years of nursing experience; hospice, palliative care, or home health experience preferred
- Strong knowledge of pain management, symptom control, and end-of-life care best practices
- Excellent communication and interpersonal skills with patients, families, and care teams
- Ability to work independently and manage a patient caseload in a field-based environment
- Valid driver's license and reliable transportation for home visits in the Fredericksburg, VA area
- Compassionate, patient-centered approach with emotional resilience in sensitive care situations
